UHD students can request or check the status of transcript evaluation by submitting the form below with their assigned UHD-ID number (900-XX-XXXX).
Please note the following:

• Transcript evaluation is available only for students that have been accepted to UHD.
• Only official transcripts can be evaluated by the Office of Admissions.
• If you attended a college or university outside of the United States, your transcripts must be evaluated by a credential evaluation service recognized by UHD prior to applying for admissions to the university. This form may only be used for evaluation of domestic transcripts.
