Requests for copies require the completion of a Texas Public Information Act Request Form. Law Enforcement entities are exempt from this requirement.

To request a copy of an accident report, incident report and/or information report, come by the University Police Department, Suite 118-North in the One Main Building, and complete a Texas Public Information Act Request Form. Processing of the form can take up to ten (10) business days.

Make sure to sign the form. In order to ensure that your request is a valid request under the Texas Public Information Act, please hand-deliver or mail the completed form to the UH-Downtown Police Department.

Completed forms are forwarded to the UH System Office of General Counsel for that office to determine, in accordance with applicable Texas law, what, if any, of the information requested will be released. An attorney from the UH System Office will then contact you in the time allowed under Texas law.
