Requests for copies require the completion of aTexas Public Information Act Request Form. Law Enforcement entities are exempt from this requirement.
To request a copy of an accident report, incident report and/or information report, please visit the University of Houston-Downtown Police Department, and complete a Texas Public Information Act Request Form. Processing of the form can take up to 10 business days. Be sure to sign the form.
To ensure that your request is a valid request under the Texas Public Information Act, please hand-deliver or mail the completed form to the UHD Police Department. See location and mailing address:

Completed forms are forwarded to the UH System Office of General Counsel for that office to determine, in accordance with applicable Texas law, what, if any, of the information requested will be released. An attorney from the UH System Office will then contact you in the time allowed under Texas law. Requests for copies will not be honored unless the Texas Public Information Act Request Form is signed and completed.
