Information Technology Forms


Information Technology account and request forms are listed below. All account forms must be completed and submitted with the appropriate departmental authorization signatures to the Information Technology office located in the Main Building, Room S-700. Forms submitted without the appropriate signatures will delay the request process. All requests require at least one week for processing.

General Computer Account Access

Please contact your hiring department to initiate this account.

New faculty, staff, adjunct and part-time employees general computer accounts are initiated by the hiring department. Please contact your hiring department for more information.

The general computer account provides all employees access to the following: UHD Computer Network; Exchange email; Blackboard; Remote Desktop Connection; VPN; UHD Wireless.

 

ATS Application Development Request

Use this form to request a new development project, modification to an existing program/report, or research.

Use this form to request access to the Baner Student Record System. This form requires appropriate departmental authorization and training.

 

Blackboard Development Course

Use this form to request a new or continuing supplemental Blackboard course.

Use this form to request departmental email account.

 

Fortis Account Access Form Adobe Acrobat Reader Icon - Download Link

Use this form to to request access to document imaging system. This form requires appropriate departmental authorization and training.

 

IT Grant Application ms word icon

 

This form is for full-time faculty member who would like to submit an IT Grant proposal. This will require approval of the Department Chair.

Multimedia Equipment Reservation Form

Use this form to reserve instructional equipment from Multimedia Services.View the equipment that is available.

 

Multimedia Service & Repair Form

Use this form to request service or repair of multimedia equipment. If your problem requires immediate attention please call the Help Desk at (713) 221-8031 (x3000).

 

Telecommunications Authorization Form Adobe Acrobat Reader Icon - Download Link

This form must be submitted if you need to add, change or discontinue any of the following telecommunication services: telephone connection, data network connection; video connection, telephone equipment transfer, telephone upgrade, long distance (FAC) code or calling card, fax line, telephone and voice mail features.

Complete this form to request access to the UHD Network. Vendors must comply with all applicable UHD policies, practice standards and agreements.

 

Website Departmental Access Form Adobe Acrobat Reader Icon - Download Link

Use this form to request access to create new webpage(s) or modification to existing webpage(s) for a departmental website. This form requires appropriate departmental authorization.

 

Website Personal Home Page Access Form Adobe Acrobat Reader Icon - Download Link

Use this form to request access to create a personal home page at UHD.

 

Guest Network Account Request Form Adobe Acrobat Reader Icon - Download Link

This form is for guests of the University who will need temporary access to the network. This form requires signature of University sponsor.
