Announcements:
Returning students who are registered with Disability Services, don't forget to drop off course syllabi for each course you are requesting academic accommodations.
ODS provides free disability related testing in the areas of Cognitive, Psychological and Attention Deficit/Hyperactivity (ADHD/ADD) disorders for any currently enrolled UHD student. Please contact the Office of Disability Services for more information.

Release of Student Information
Students who wish to give permission to receive or exchange information with the ODS must complete the Authorization to Release form.
Students who wish to give permission to a medical or professional provider to receive or exchange information with ODS must complete the Authorization to Release HCP form.
Students are required to contact the Office of Disability Services to request the release forms.
