A Program Operator may not employ an individual in a position involving contact with a campus program for minors unless:
- The individual successfully completes the UHS Campus Program for Minors training and examination program on sexual abuse and child molestation and documentation is available to verify successful completion (this is the highly preferred option), or
- The individual submits to the Program Operator or the Program Operator has on file documentation that verifies the individual within the preceding two years successfully completed the training and examination program on sexual abuse and child molestation either offered by UHS or a program on the Texas Department of State and Health Services Approved Training and Examination Programs.
The Program Operator is responsible for ensuring the following items occur prior to any Campus Programs for Minors being conducted by a third party (i.e. any vendor or individual not employed by UHD) on the UHD campus:
Step 1.
Complete the Spreadsheet for Campus Programs for Minors and forward it to the Coordinator of Campus Programs for Minors. If UHD employees will be working with minors in the program:
- Include the names of the employees,
- Forward the spreadsheet electronically to the Coordinator of Campus Programs for Minors,
- Have each employee complete a (SSQ) Security Sensitive Questionnaires, and
- Hand carry a copy the SSQs to ESO (Invonne Montalbano or April Frank)
Step 2.
The third party should forward to the Program Operator:
Within 15 days of the program, the vendor must submit to the Texas Department of State Health Services (TDSHS):
- The Department of State Health Services - Campus Program for Minors Information Form indicating each individual who will be working or volunteering at the program has completed the Sexual Abuse and Child Molestation Training from an Approved Training and Examination Programs.
- If the third party takes the UH System training, ESO will provide the results of the criminal background check and the dates each individual successfully completed the UH System training. Attach this information to the form.
- The course information needed to complete the form is as follows:
- UH System Training Course Name: University of Houston System Child Protective Training
UH System Course Approval Number: CPM12-0072
- Harris County Code: 101
The third party should forward to the Program Operator:
- A copy of the completed Department of State Health Services - Campus Program for Minors Information Form, and
- Verification that the form has been forwarded to TDSHS..
Step 3.
The third party must certify in writing to UHD that no individual who will be working or volunteering at the Campus Program for Minors has any felony criminal convictions or any crime(s) related to abuse, neglect, exploitation, assault, sexual offense, or any offense involving a minor.
- A copy of the fully executed contract between the third party and UHD with the Addendum for contracts involving Campus Programs for Minors serves as the required certification.
Step 4.
The third party must agree to indemnify and hold harmless the component university from any and all liability and claims arising from the program.

Step 5.
The third party must agree to maintain insurance acceptable to the component university including listing the component university as an additional insured and providing a certificate of insurance before the program occurs.
- The third party must provide a Certificate of Insurance that meets the contracted insurance requirement.
- The Certificate of Insurance must show the University of Houston-Downtown as an additional insured.
- The Certificate of Insurance must cover the contract period.
The Program Operator must obtain written approval from the Coordinator of Campus Programs for Minors to conduct the program.

Program Operator
Seven days prior to the beginning of the program, the Program Operator (or college/department that facilitates the contract with the third party) must forward the following items to the Coordinator of Campus Program For Minors:
- A copy of third party's completed Department of State Health Services - Campus Program for Minors Information Form along with some form of verification that the form has been forwarded.
- A copy of the fully executed contract between the third party and UHD with the Addendum.
- A Certificate of Insurance that meets the requirement in Step 4 above.
[Note: Incomplete packages will not be processed.]
Keep a copy of the entire file with all supporting documentation for a period that is to be determined and will be communicated at a later date.
Coordinator of Campus Programs for Minors
The Coordinator of Campus Programs for Minors will:

- Review the items listed above.
- Verify all required items in the package meet the program requirements.
- Provide written and dated notice to the Program Operator of approval to conduct the program.
