This page contains forms and documents used in the Campus Programs for Minors organized into categories that will allow quick access for the Program Operator or department facilitating contracts with a third party conducting programs for UHD that involve minors. Policies and Regulations

SAM 05.C.01 - Campus Programs For Minors

 

The UH System policy developed to both:

  • Ensure that the Campus Programs for Minors are conducted in a manner that is as safe as possible, and

  • Comply with the legal requirements of Texas Education Code § 51.976

 

 

Texas Education Code § 51.976

 

Requires individuals who will have contact with minors at certain limited campus programs to receive specific training and testing on preventing sexual abuse and child molestation.  The training must be documented on the Campus Program for Minors Information Form and forwarded with supporting documentation to the Coordinator of Campus Programs for Minors.  The Coordinator will provide written and dated notice to the Program Operator of approval to conduct the program.

 

Section 51.967

 

The section of the law that specifically addresses the training and examination on warning signs of sexual abuse and child molestation for employees of camp programs for minors.

Campus Program for Minors Information Form

 

UH System Training Course Name: University of Houston System Child Protective Training

 

UH System Course Approval Number: CPM12-0072

 

Harris County Code: 101

 

 

This form provides information on a specific Campus Program for Minors and documents that each individual who will be working or volunteering at the program has completed the Sexual Abuse and Child Molestation Training from an Approved Training and Examination Program.

This document must be forwarded to the Texas Department of State Health Services (TDSHS) by the Coordinator of Campus Programs for Minors or the third party vendor as appropriate. The Coordinator will review the document and supporting information to verify the requirements have been met, forward documents to the TDSHS where appropriate, and provide written and dated notice to the Program Operator of approval to conduct the program.

 

Training and Examination Program Application

 

If the third party chooses to develop their own training and examination program, this application is used to apply to the State of Texas for review and approval of the developed training program.

The instructions for submitting the required materials and non-refundable fees are included on the application.

 

Approved Training and Examination Programs

 

This is the current list of training and examination programs that have been approved by the Texas Department of State Health Services (TDSHS). The list includes the name of the organization, contact information, the name of the program, the approval number and the expiration date of the approved program.

UHS Training Information Sheet

 

A UH System information sheet developed to provide instructions to access the Child Protection Training. The document contains information on:

  • How to enroll in the training via PASS,
  • Login information (accessing Blackboard and locating the training),
  • Blackboard troubleshooting, and
  • How the individual will know that he/she has passed the course.

 

POI (Person of Interest) Form

 

This form is used to request a PeopleSoft (PS) ID for a non-UHD employee or non-UHD student. PS IDs are needed for accessing many campus services, including NARS.

Instructional guide on how to set up non-UHD employees or non-UHD students as persons of interest (POI) into the PeopleSoft system.

A system which allows designated employees to initiate computer account access for new/prospective employees.

This is the PeopleSoft link to the Child Protection Training and examination on Blackboard. Once an individual has completed the self-enrollment process, it will take 24 hours for the training to show in their training queue.

Security Sensitive Position Questionnaires (SSQ)

 

Form used to gather background information about the prospective employee or person of interest from a consumer reporting agency in connection with the employment screening process.

 

Under "Please select report(s)" check "Criminal Background Check".

 

This document contains private information pertaining to the individual. It should be secured at all times and hand delivered to the VP of Employment Services and Operations (Ivonne Montalbano) or the Records & Business Administrator (April Frank).

Campus Programs Conducted by UHD

Campus Programs for Minors Spreadsheet

 

Completion of this spreadsheet by the Program Operator is the first step in the process to obtain written and dated notice of approval to conduct the program. It should be completed at least 21 days before the program begins.

This spreadsheet provides key information regarding the program (e.g. name, type, description, location, dates, number and age of participants) and information on the program staff teaching and interacting with the students.

 

The completed spreadsheet should be forwarded to:

  • The Coordinator of Campus Programs for Minors, and
  • Hand delivered to the VP of Employment Services and Operations (Ivonne Montalbano) or the Records & Business Administrator (April Frank) along with the Security Sensitive Questionnaires (SSQ) which contains private information pertaining to the individual and should be secured at all times.

This Department of State Health Services form documents the program and the individuals who have been trained, successfully tested and as a result are able to be employed by the Program Operator for the specified Campus Program for Minors.

 

This form must be completed by the Program Operator and turned in to the Coordinator of Campus Programs for Minors along with the supporting documentation that validates the information for the individuals listed on the form.

 

The Coordinator of Campus Programs for Minors must review this package to verify completion. The Coordinator will then provide :

  • Submit the Department of State Health Services - Campus Program for Minors Information Form to the Texas Department of State Health Services (TDSHS), and
  • Provide written and dated notice to the Program Operator of approval to conduct the program.

Campus Programs Conducted by Third Parties

Approved Training and Examination Programs

 

This is the current list of training and examination programs that have been approved by the Texas Department of State Health Services (TDSHS). The list includes the name of the organization, contact information, the name of the program, the approval number and the expiration date of the approved program.

 

Department of State Health Services - Campus Program for Minors Information Form

 

This Department of State Health Services form documents the program and the individuals who have been trained, successfully tested and as a result are able to be employed by the Program Operator for the specified Campus Program for Minors.

Within 15 days of the program, the vendor must submit to the Texas Department of State Health Services (TDSHS):

  • If the third party takes the UH System training, the information needed to complete the form is as follows:
      • UH System Training Course Name: University of Houston System Child Protective Training
      • UH System Course Approval Number: CPM12-0072
      • Harris County Code: 101 Link to Texas County County Numbers

 

The third party should forward to the Program Operator:

  • Verification that the form has been forwarded to TDSHS..

The Program Operator will also need:

  • A copy of the fully executed contract between the third party and UHD with the Addendum , and
  • A Certificate of Insurance that meets the contracted insurance requirements, shows the University of Houston-Downtown as a "Named Insured", and covers the period of the contract.

 

The Program Operator will need all of these documents to obtain written approval from the Coordinator of Campus Programs for Minors. Written approval is required to conduct the program on the UHD campus.

 

Addendum for Contract

 

This Addendum must be attached to every contract with a third party who will be conducting (running, supervising and operating) a campus program for minors on the UHD campus.
