Nomination Form & Instructions for 2012

Please join us in honoring our staff members for their dedication and service to UHD. By nominating someone for one of the merit awards, you are giving them special recognition for their hard work. You may either nominate an individual for an award or write a letter in support of a nomination package being prepared by someone else.

The nomination process is simple. Prepare a package including a nomination form (use the attached nomination form) and include at least two additional statements in support of your nominee (using the attached statement of support form). Please send sealed nomination packages by campus mail to: Doug teDuits, Employment, Training, Campus Relations and Affirmative Action Officer, room North-276.

PLEASE NOTE: In order to be considered for one of these awards, each nomination package must be complete, consisting of a nomination form plus separate statements in support of the nominee from at least two (but no more than five) additional people. Incomplete nomination packages will not be reviewed by the board. All nominations and supporting letters must be typed and then printed out in hard-copy form, and be submitted altogether as one complete nomination package. All nomination forms and letters of support MUST be hand signed; digital signatures will not be accepted.

The staff awards ceremony will be held on Friday, June 1st in the auditorium.

The Awards

Staff Excellence (Formerly Merit Award):

One nominee from each of the three job categories is selected as a Staff Excellence award winner. The three job categories are:

• Technical Services & Crafts (which includes: Facilities Management, the Police Dept. & IT)
• Office & Clerical (which includes: Administrative and office staff that are hourly employees)
• Professional Staff (which includes all salaried employees)

President’s Choice Award (Formerly Distinguished Award)

The President’s Choice Award is a two-step process. Out of the three Staff Excellence Award finalists, from the three job categories listed above, one is selected as the President’s Choice Award winner.

Eligibility

Nominees must meet the following criteria. Must demonstrate the qualities of:

• Continued productivity in excess of normal job description
• Initiative resulting in benefit to UHD
• A positive attitude that affects fellow co-workers, and the general public
• Dependability, responsibility and stability
• Must have at least five years of service with UHD in a full-time position
• Must be nominated with a completed and signed nomination package
• Must not have been a recipient of the President’s Choice or Staff Excellence Award in the last five years.

PLEASE NOTE: Current members of the Staff Council Executive Committee are not eligible. UHD Administration is not eligible.

Selection Process:

The Awards Review Board (ARB), which is appointed by the Staff Council President, selects the winners from the eligible nominees. The ARB is comprised of former Distinguished (now, President’s Choice Award) and Merit Award winners, an ESO representative, members of staff council, and professional (non-hourly) staff members.
