Student Problem-Solving Form

 

 

The purpose of this form is to provide students with an avenue to submit a concern or complaint about anything at UHD for which no other specific process exists. This form should not be used to submit complaints regarding grades, acceptance of transfer credit, information in academic records, academic honesty decisions, sexual harassment, or sexual assault. Please see the Student Handbook for the grievance procedures related to those areas. Submitting this form does not substitute for following those procedures.

Emergency situations should be reported to the University Police at 713-221-8911.

Once you have submitted a Problem-Solving Form, you can expect to be contacted by the close of the next business day.
