The University of Houston – Downtown is committed to addressing behavior that violates our campus community standard. This page contains resources for faculty members who have questions about the student conduct process, campus resources, and how to report concerning or threatening behaviors.

There are several reporting forms that assist us in addressing student behavior that violate community standards: including academic, concerning, disruptive, and threatening behaviors to the educational environment (all modalities) on and off-campus.

Reporting Forms

Academic Dishonesty Form  Faculty should submit this form for reporting student issues related to plagiarism, cheating, and falsification of information within the classroom environment. Faculty are responsible for submitting a copy of the report to the student, their department chair, and the Dean of Students Office.

Care Form - Campus community members are encouraged to submit this form for reporting student issues related to the death of a loved one, hospitalization, serving as the primary caregiver for a loved one, incarceration, physical health challenges, difficult situations at home, and any other situations.

Student Conduct Incident Report Form - Campus community members are encouraged to submit this form for reporting student issues related to any violations of campus policy such as alcohol, drugs, physical harm, disruptive activity, and other violations.

Sexual Misconduct Form - Campus community members are required to submit this form for reporting student issues related to sexual misconduct such as non-consensual fondling, sexual assault, groping, domestic violence, stalking, and other unwanted advances and touching. Matters related to sexual misconduct are managed by the Title IX Office.

Discrimination & Harassment Form - Campus community members are encouraged to submit this form for reporting student issues related to harassment or discrimination of a protected class experienced or witnessed.

Student Complaint Form - Campus community members are encouraged to direct students to submit this form to report issues related to any grievances experienced, processes, or procedures not followed by university departments or personnel.​
